We are happy to verify your health care insurance benefits to check what type of services will be covered at our facility. Please fill out the form below completely and accurately. We will check the benefits as quickly as possible. 

All information is kept confidential according to your rights to privacy, which are fully protected by HIPAA, The Health Insurance Portability and Accountability Act. 

We accept: Private Pay (check/ credit cards) and all PPO Out Of Network Insurances . We do not accept Medicaid or MediCal. 

We accept: Private Pay (cashier check/credit cards, wire transfer) and PPO Out Of Network Insurances.
